These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Brussels Sprout interacts with whole categories of medication. Acetaminophen, Caffeine, Pyrilamine falls into 3 of those categories, so it can interact with Brussels Sprout in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Acetaminophen, Caffeine, Pyrilamine
Acetaminophen, Caffeine, Pyrilamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Brussels Sprout interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters for your Midol Max Strength: the caffeine in it is handled by a liver enzyme called CYP1A2. Animal studies hint that Brussels sprouts might speed that enzyme up a little, which could clear caffeine out of your system a bit faster. In plain terms, you might notice slightly less of the caffeine pick-me-up, like a touch less alertness. The acetaminophen and pyrilamine pieces aren't really affected this way.
Honestly, this is weak, theory-level evidence, and a normal serving of Brussels sprouts on your plate is very unlikely to change how this works for you. If you're eating large amounts or taking concentrated supplements, mention it to your pharmacist, but there's no need to worry about your veggies here.

GLUCURONIDATED DRUGS
2 of 3Acetaminophen, Caffeine, Pyrilamine also falls under GLUCURONIDATED DRUGS.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist for your Midol Max Strength. The acetaminophen part of this combo gets cleared by your liver through a process called glucuronidation. Brussels sprouts (and cabbage) seem to gently speed that pathway up, so in theory your body might clear the acetaminophen a little faster. That could mean slightly weaker or shorter pain relief, not extra side effects. Acetaminophen isn't a prodrug, so there's no flip here.
Honestly, this is a mild, mostly theoretical concern. Eating Brussels sprouts at dinner is very unlikely to noticeably change how your menstrual pain reliever works. If you ever feel like the medicine isn't holding up, chat with your pharmacist rather than changing your dose on your own.

ACETAMINOPHEN (Tylenol, others)
3 of 3Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the deal with your Midol Max Strength. The acetaminophen part is the piece that interacts with Brussels sprouts. Compounds in cruciferous veggies like Brussels sprouts and cabbage can rev up one of the pathways your liver uses to clear acetaminophen (a process called glucuronide conjugation). In clinical research, a diet heavy in these vegetables lowered acetaminophen levels by up to about 16 percent.
In plain terms, the acetaminophen may not work quite as well for your pain or cramps. This is a modest effect, and a normal serving of Brussels sprouts at dinner is unlikely to make a real difference. The caffeine and pyrilamine pieces aren't affected here. If you eat a lot of these veggies and notice less relief, chat with your pharmacist.

- Heart health support
- Cancer-prevention interest
Brussels sprouts are a nutritious cruciferous vegetable rich in fiber, vitamin C, vitamin K, and plant compounds called glucosinolates.
