Why this applies to Estradiol, Norethindrone Acetate

Estradiol, Norethindrone Acetate falls under ESTROGENS — the group Caffeine interacts with.

Theoretically, estrogens might increase the levels and adverse effects of caffeine.
Estrogen inhibits caffeine metabolism.

Here's something a lot of folks don't realize: the estrogen in Activella (estradiol with norethindrone) can slow down how your body clears caffeine. The estrogen part actually blocks the liver enzyme that breaks caffeine down, so the caffeine you drink tends to hang around longer and build up a bit more than usual.

What does that look like in real life? You might feel more jittery, notice your heart racing or pounding, feel anxious, or have a harder time falling asleep, basically caffeine's usual effects, just turned up a notch. This is a moderate, fairly likely interaction, but it's easy to manage. You don't need to stop your medication. If you notice these symptoms, simply cutting back on coffee, tea, or energy drinks usually does the trick. Chat with your pharmacist if you're unsure.

CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

2 of 2

Estradiol, Norethindrone Acetate also falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's something a little different from the usual concern. With your Activella (estradiol and norethindrone), it isn't the hormone that builds up, it's the caffeine. The estrogen part can gently slow down a liver enzyme called CYP1A2, which is the same enzyme that clears caffeine out of your body. When caffeine hangs around longer, your usual cup of coffee may hit a bit harder.

In real life, that might mean feeling more jittery, a racing or pounding heart, trouble sleeping, or feeling on edge. Keep in mind this is rated minor and is based only on lab-type evidence, so most people won't notice much. If you feel wired lately, simply easing back on coffee, tea, or energy drinks usually does the trick. Check with your pharmacist if you're unsure.

From our pharmacist’s overview on the Estradiol, Norethindrone Acetate page

“Activella pairs estradiol with norethindrone as hormone therapy, and because the treatment depends on steady hormone levels in the blood, supplements that raise or lower those levels warrant real scrutiny. Three of the 212 interacting ingredients rate major, and all three hit that weak point: St.”
