Caffeine and Ethiny Estradiol, Levonorgestrel

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Caffeine interacts with whole categories of medication. Ethiny Estradiol, Levonorgestrel falls into 3 of those categories, so it can interact with Caffeine in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ESTROGENS

1 of 3

Why this applies to Ethiny Estradiol, Levonorgestrel

Ethiny Estradiol, Levonorgestrel falls under ESTROGENS — the group Caffeine interacts with.

Theoretically, estrogens might increase the levels and adverse effects of caffeine.
Estrogen inhibits caffeine metabolism.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's a fun twist on this pairing: it's actually the birth control pill affecting the caffeine, not the other way around. The estrogen in your Microgynon or Logynon (ethinyl estradiol) can slow down the liver enzyme that clears caffeine from your body. That means your morning coffee, tea, or energy drink may stick around longer and hit a little harder.

In real life, you might notice more jitteriness, a faster heartbeat, trouble sleeping, or feeling wired if you drink your usual amount. This doesn't make your pill less effective at preventing pregnancy. If you feel extra buzzed, simply easing back on caffeine usually does the trick. Have a quick chat with your pharmacist if it bothers you.

CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

2 of 3

Ethiny Estradiol, Levonorgestrel also falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's a nice twist on how this pairing works. Your birth control pill (the estrogen and levonorgestrel in Logynon or Microgynon) isn't the one that gets affected here. Instead, the estrogen part can gently slow down the liver enzyme (CYP1A2) that normally clears caffeine out of your system. So your morning coffee or tea may stick around a little longer than usual.

In real life that might mean you feel more wired, jittery, restless, or have trouble sleeping, or notice your heart racing a bit. This is a minor, mostly theoretical effect, so most people won't notice much. If caffeine feels stronger than it used to, simply ease back on it and mention it to your pharmacist or doctor.

CONTRACEPTIVE DRUGS

3 of 3

Ethiny Estradiol, Levonorgestrel also falls under CONTRACEPTIVE DRUGS.

Theoretically, contraceptive drugs might increase the levels and adverse effects of caffeine.
Oral contraceptives decrease the rate of caffeine clearance by 40-65%.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's a nice twist on the usual interaction question: it's not your birth control pill (Logynon or Microgynon) that gets affected here, it's the caffeine. The estrogen in your pill slows down how fast your body clears caffeine, by roughly 40 to 65 percent. In plain terms, that morning coffee can hang around in your system longer than it would otherwise.

What that may mean for you: caffeine can feel a bit stronger and last longer, so you might notice more jitteriness, a racing heart, or trouble sleeping if you have a lot of it. This is considered minor and is partly theoretical, so there's no need to panic. If you feel wired, simply easing back on coffee, tea, or energy drinks usually does the trick. Your pill itself still works as intended. Any questions, your pharmacist is happy to help.
