Esomeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the short version: your body relies on a liver enzyme called CYP2C19 to break down esomeprazole (Nexium). CBD slows that enzyme down, so the medicine can clear out more slowly and build up a bit higher in your blood. Esomeprazole isn't a prodrug, so this means a little more drug effect, not less.

The good news is that esomeprazole is a pretty gentle, well-tolerated medicine. Even with higher levels, the most you'd likely notice is mild stuff like headache, nausea, gas, or diarrhea. This isn't dangerous for most folks, but since the science behind it is solid, it's worth a quick chat with your pharmacist or doctor before pairing the two, especially if you take CBD daily.

Here's the gist for this pair: your CBD is actually the thing that may be affected, not Nexium. Esomeprazole slows down a liver enzyme called CYP2C19, and that same enzyme helps your body break down CBD. So when you take both, your CBD could clear out a little slower and build up somewhat, which may bump up its effects and side effects, like drowsiness, lightheadedness, dry mouth, or an upset stomach.

Now, the honest caveat: this is a theoretical concern based on how these compounds work, not something proven in real-world studies. It's likely mild for most people. Still, if you notice extra sleepiness or feel 'off' after adding CBD, loop in your pharmacist or doctor before changing anything.

Here's the gist for Nexium (esomeprazole): your liver uses enzymes to break it down, and one of those is called CYP3A4. CBD can slow that enzyme down a bit. In theory, that means a little more esomeprazole could linger in your system than usual.

The good news is that esomeprazole is generally a forgiving, well-tolerated medicine, and it's mostly cleared by a different enzyme (CYP2C19) more than 3A4. So even if levels nudged up slightly, you'd most likely just notice the typical mild things, like headache, nausea, or some gas. What Is Esomeprazole Used For?

Brand name: Nexium

Esomeprazole treats problems caused by excess stomach acid. It's used to heal erosive esophagitis (damage to the esophagus from acid), control GERD (gastroesophageal reflux disease) symptoms like heartburn, and protect the stomach lining in people at risk for ulcers caused by NSAIDs (anti-inflammatory pain relievers like ibuprofen).

It's also used as part of a three-drug combination to wipe out H. pylori — a stomach bacteria linked to ulcers — and for people with rare conditions that cause the stomach to overproduce acid, such as Zollinger-Ellison syndrome.
