In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for your Opsumit (macitentan). Your liver uses an enzyme called CYP3A4 to help clear this medicine from your body. CBD can slow that enzyme down, which means macitentan may not break down as quickly and can build up to higher levels than usual.

Macitentan is the active drug (not a prodrug that needs switching on), so more of it in your system can mean stronger effects and a greater chance of side effects like swelling in your legs or feet, headache, a stuffy nose, anemia, or feeling unusually tired. The evidence that CBD blocks this enzyme is solid, so it's worth a quick chat with your pharmacist or doctor before combining them. Please don't change anything on your own.

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to lower the chance of being hospitalized because of PAH.

In the key clinical study, macitentan was used in adults with mainly moderate-to-severe PAH symptoms. Patients had PAH caused by unknown or inherited causes, connective tissue disorders, or congenital heart disease with repaired shunts.
