Here's the gist with Macitentan (Opsumit) and ashwagandha. Macitentan already relaxes and widens blood vessels in your lungs, which can lower your blood pressure a bit. Ashwagandha may nudge blood pressure down too, so taking them together could, in theory, add up and leave you feeling lightheaded, dizzy, or wiped out, especially when you stand up quickly.

I want to be honest about the evidence though: this concern comes only from animal studies, not human trials, so it's more of a theoretical caution than a proven problem. Still, since Macitentan is a serious medication for a serious condition, please check with your pharmacist or doctor before adding ashwagandha rather than starting it on your own.

Here's the thing worth knowing about pairing ashwagandha with macitentan (Opsumit). This isn't about ashwagandha changing how macitentan works or how much ends up in your blood. The concern is that both can be hard on the liver. Macitentan, like other drugs in its class, can raise liver enzymes in some people, and ashwagandha has been tied to rare cases of liver injury on its own.

So the worry is a double hit to the same organ. Watch for things like yellowing of the skin or eyes, dark urine, belly pain on your right side, nausea, or unusual tiredness. To be clear, this is a theoretical concern based on how each acts, not proven for this pair. Since you likely already get liver bloodwork on macitentan, check with your doctor or pharmacist before adding ashwagandha.

Here's the basic idea with this pairing. Your body clears macitentan (Opsumit) using a liver enzyme called CYP3A4. Some lab studies suggest ashwagandha may speed that enzyme up a bit. If that happened in real life, your system could break down macitentan faster, leaving less of it to do its job of relaxing the blood vessels in your lungs.

In plain terms, the theoretical worry is a weaker effect, which for PAH could mean your breathing or exercise tolerance not staying as well controlled. That said, this is only based on test-tube research, not studies in people, so it's flagged as minor. Since macitentan is a serious medication for a serious condition, I'd just run the ashwagandha idea past your doctor or pharmacist first.

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a serious condition where the blood pressure in the arteries supplying the lungs is abnormally high. The goal is to slow down how quickly the disease gets worse and to lower the chances of being hospitalized because of it.

In clinical studies, it was shown to be effective in adults with mostly moderate-to-severe PAH symptoms. Patients in those studies had PAH caused by unknown or inherited reasons, connective tissue disorders, or congenital heart disease with repaired heart shunts.
