Avacopan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for Avacopan (Tavneos). Your body relies on an enzyme called CYP3A4 to help process this drug. Lab studies hint that cascara sagrada might gently speed up that enzyme, which in theory could clear Avacopan a little faster and leave slightly less of it working in your body. For a medication you're taking to calm your immune system and protect your blood vessels, weaker drug levels aren't something you'd want.

That said, I want to keep this in perspective. This concern comes only from test-tube research, and even there cascara's effect was much weaker than known strong inducers. So it's more of a theoretical caution than a proven problem. Still, if you're considering cascara, please check with your pharmacist or doctor first.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Avacopan Used For?

Brand name: Tavneos

Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.

Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
