Avapritinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for your cancer medicine. Avapritinib (Ayvakit) is cleared from your body by a liver enzyme called CYP3A4. Cascara sagrada may gently speed that enzyme up, at least according to early lab studies. If that happened in real life, your body could clear avapritinib a little faster, which in theory could leave less of it working against your tumor or mast cell disorder.

I want to be honest about how thin this evidence is. It comes only from test-tube research, and the effect looked much weaker than known enzyme boosters like rifampin. So for most people this is more of a theoretical caution than a proven problem. Still, avapritinib is a serious drug where steady levels matter, and cascara isn't a gentle laxative. Please check with your oncology team or pharmacist before pairing them.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Avapritinib

What Is Avapritinib Used For?

Brand name: Ayvakit

Ayvakit (avapritinib) is used to treat adults with unresectable (cannot be surgically removed) or metastatic (spread to other parts of the body) gastrointestinal stromal tumor (GIST) — but only in patients whose tumor has a specific genetic change called a PDGFRA exon 18 mutation, including the D842V mutation. A test is needed to confirm this mutation before starting treatment.

Ayvakit is also used to treat advanced systemic mastocytosis (AdvSM) — a serious condition where too many mast cells build up in the body — including its subtypes: aggressive systemic mastocytosis (ASM), systemic mastocytosis with an associated blood cancer (SM-AHN), and mast cell leukemia (MCL). It is also approved for indolent systemic mastocytosis (ISM), a less aggressive form. Ayvakit is not recommended for patients with AdvSM or ISM who have very low platelet counts (below 50 × 10⁹/L).
