Crinecerfont falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the short version: Crinecerfont (Crenessity) is handled in your body partly by a liver enzyme called CYP3A4. Cascara sagrada, in lab dish studies only, seems to nudge that enzyme to work a little faster. If that held true in people, it could clear crinecerfont out a bit quicker and make it slightly less effective at doing its job, which is helping control the hormone problems in congenital adrenal hyperplasia.

I want to be honest, though: this is theoretical and comes only from test-tube research, and the effect looked weak compared to real enzyme boosters like rifampin. So it's a minor concern, not an alarm. Still, since consistent crinecerfont levels matter, please run any cascara use by your pharmacist or doctor first.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.
