Lonafarnib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the basic idea: Lonafarnib (Zokinvy) is cleared from your body mainly by a liver enzyme called CYP3A4. Some lab studies hint that cascara sagrada might speed that enzyme up a little, which in theory could lower lonafarnib levels and make it work less well over time. Since lonafarnib is taken for a serious condition (progeria), getting full benefit from each dose really matters.

I want to keep this in perspective, though. This is only a theoretical, test-tube finding, and the effect was far weaker than known enzyme boosters. It hasn't been shown in real patients. Still, cascara has its own risks, like cramping and electrolyte loss. Please check with your pharmacist or doctor before pairing the two.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Lonafarnib

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
