Tepotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist if you're taking Tepmetko (tepotinib) and thinking about cascara sagrada. Your body clears tepotinib using a liver enzyme called CYP3A4. Lab studies hint that cascara may gently speed that enzyme up, which could clear tepotinib a little faster and, in theory, make it work less well against your lung cancer. Tepotinib is an active drug (not a prodrug), so faster clearance means weaker effect, not stronger.

Now, the honest part: this is only seen in test-tube research, the effect is much weaker than known inducers like rifampin, and it's rated a minor, possible concern. I wouldn't lose sleep over it, but since tepotinib is cancer treatment, please run any cascara use by your oncology team or pharmacist first.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

Tepmetko (tepotinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — lung cancer that has spread — whose tumors carry a specific genetic alteration called a MET exon 14 skipping mutation. A test on tumor tissue or blood is required to confirm this mutation before treatment begins.

This is a targeted therapy, meaning it works against a specific molecular driver of cancer growth rather than attacking all rapidly dividing cells.
