Trimetrexate Glucuronate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Trimetrexate (Neutrexin) is a potent medication used to treat serious infections like Pneumocystis pneumonia, and your body partly clears it through a liver enzyme called CYP3A4. The thinking here is that cascara sagrada might gently speed up that enzyme, which could clear trimetrexate a little faster and slightly weaken how well it works against the infection. Since trimetrexate isn't a prodrug, faster clearance points toward less drug effect, not more.

Honestly, this is a very theoretical concern. It comes only from lab studies, the effect on the enzyme was far weaker than known boosters, and it's rated minor. Still, this is a hospital-administered drug for serious illness, so let your care team know about any supplements you take.
