Corydalis Yanhusuo and Saxagliptin, metformin XR

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Corydalis Yanhusuo interacts with whole categories of medication. Saxagliptin, metformin XR falls into 2 of those categories, so it can interact with Corydalis Yanhusuo in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIDIABETES DRUGS

Why this applies to Saxagliptin, metformin XR

Saxagliptin, metformin XR falls under ANTIDIABETES DRUGS — the group Corydalis Yanhusuo interacts with.

Theoretically, Corydalis yanhusuo may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Corydalis yanhusuo contains berberine. Clinical research shows that berberine may lower blood glucose levels. Theoretically, Corydalis yanhusuo might also lower blood glucose levels.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the simple version: your Kombiglyze (saxagliptin plus extended-release metformin) is already working to bring your blood sugar down. Corydalis yanhusuo may contain compounds, including berberine, that can also nudge blood sugar lower on their own. Stack the two together and the concern is that your numbers could dip too far, which we call hypoglycemia.

If that happened, you might feel shaky, sweaty, dizzy, confused, or unusually hungry. I want to be honest though: this interaction is theoretical, based on how these compounds behave, not on solid human studies pairing the two. So it's worth a conversation, not a panic. Please check with your pharmacist or doctor before adding Corydalis, and keep an eye on your home readings.

METFORMIN (Glucophage)

Theoretically, Corydalis yanhusuo might increase the therapeutic and adverse effects of metformin.
Corydalis yanhusuo contains berberine. In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin. It is unclear if Corydalis yanhusuo might have this same effect.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your Kombiglyze combines saxagliptin with extended-release metformin to help lower blood sugar. The concern here is mostly about the metformin part. Corydalis may contain berberine, and in lab and animal studies berberine slowed how quickly the body cleared metformin, letting more of it build up. More metformin in your system could mean a stronger blood-sugar-lowering effect and more of metformin's usual side effects, like nausea, stomach upset, diarrhea, or a metallic taste.

Keep in mind this is theoretical, based on pharmacology and not on solid human studies, and it's unclear whether corydalis acts the same way. Still, since your blood sugar matters, please check with your pharmacist or doctor before pairing these, and watch for signs of low blood sugar.
