Acetaminophen, Codeine, Doxylamine falls under ANTICHOLINERGIC DRUGS — the group Datura Wrightii interacts with.

Datura wrightii contains atropine and scopolamine, as well as other tropane alkaloids with anticholinergic activity. Concomitant use of Datura wrightii with anticholinergic drugs might increase anticholinergic effects and adverse effects. Some anticholinergic drugs include amantadine, atropine, belladonna alkaloids, phenothiazines, scopolamine, and tricyclic antidepressants.

Let me start with the most important thing: Datura wrightii is genuinely poisonous and is not something I'd ever recommend taking, even in tiny amounts. With that said, here's the specific concern with Mersyndol. One of its three ingredients, doxylamine, is an antihistamine that already has its own drying, sedating, anticholinergic effects. Datura adds atropine and scopolamine on top of that, so the two stack together.

In practice, that could mean a dry mouth, blurry vision, constipation, trouble urinating, a faster heartbeat, and worse, confusion, drowsiness, or hallucinations (especially since codeine is sedating too). This is based on how these compounds work rather than firm studies, but the pharmacology is sound. Please don't combine them, and talk with your pharmacist or doctor.
