Dhea and Pantoprazole

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Pantoprazole

Pantoprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Dhea interacts with.

Theoretically, DHEA might increase the levels of drugs metabolized by CYP3A4.
Some preliminary evidence shows that DHEA may inhibit CYP3A4; however, the clinical significance of this potential interaction is not known.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for combining DHEA with pantoprazole. Pantoprazole is an active acid-blocking medicine (not a prodrug), and it's partly broken down by a liver enzyme called CYP3A4. There's some early, anecdotal lab evidence that DHEA might slow that enzyme down a bit. In theory, that could let a little more pantoprazole hang around in your body.

Honestly, this is very weak evidence and the real-world importance isn't known. Pantoprazole is also a forgiving drug with a wide safety margin, so even a small bump is unlikely to cause trouble. If anything, you might notice mild things like headache, gas, or loose stools. I wouldn't lose sleep over it, but it's always smart to mention any supplement to your pharmacist or doctor so they can keep an eye on things.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Dhea

What is Dhea?

Dhea
  • Anti-aging and low DHEA levels
  • Mood and depression
  • Vaginal dryness and sexual health
  • Fertility support

DHEA is a natural hormone that the body makes and that declines with age, and it is sold as a supplement claiming many benefits. The evidence is mixed and limited for most uses, and because it acts as a hormone it can cause real side effects. Talk to a pharmacist or doctor before using it, especially if you have a hormone-sensitive condition.

About Pantoprazole

What Is Pantoprazole Used For?

Brand names: Protonix, Pantoloc, Protonix IV

Pantoprazole treats erosive esophagitis (damage to the lining of the esophagus) linked to gastroesophageal reflux disease (GERD), a long-term condition where stomach acid backs up. It is also used for pathological hypersecretory conditions, including Zollinger-Ellison (ZE) syndrome, where the stomach makes far too much acid.

Which use applies depends on the product. The oral delayed-release tablets and the delayed-release oral suspension (granules), including Protonix, cover short-term healing, maintenance of healing, and the excess-acid conditions. The intravenous (IV) forms, including Protonix I.V., are for adults with GERD and a history of erosive esophagitis for a short time, and for ZE syndrome and similar conditions.
