Moringa and Pantoprazole

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Pantoprazole

Pantoprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture with moringa and your pantoprazole. Pantoprazole is partly cleared by a liver enzyme called CYP3A4, and lab studies disagree about what moringa does to it. Some test-tube work says moringa slows that enzyme down (which could let a bit more pantoprazole build up), while other lab work says it speeds the enzyme up (which could lower your levels slightly). So we genuinely can't say which way it goes.

The good news: pantoprazole is a forgiving medication with a wide safety margin, and this is only theoretical lab data, not something seen in real patients. A small human study with a related drug showed no meaningful change. I wouldn't lose sleep over this, but mention any moringa supplement to your pharmacist, especially in concentrated doses rather than food amounts.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Moringa

What is Moringa?

  • Nutritional supplement
  • Blood sugar support
  • Cholesterol support
  • Inflammation
  • Skin conditions

Moringa is a nutrient-rich plant whose leaves are widely used as a food and supplement, especially in parts of the world where malnutrition is common. Early research hints at possible benefits for blood sugar, cholesterol, and inflammation, but the human evidence is limited and not strong enough to recommend it as a treatment. Talk with your pharmacist or doctor before using moringa, especially if you are pregnant, breastfeeding, or taking medications.

About Pantoprazole

What Is Pantoprazole Used For?

Brand names: Protonix, Pantoloc, Protonix IV

Pantoprazole delayed-release tablets and oral suspension are used in adults and children 5 years and older to heal erosive esophagitis (EE) — damage to the esophagus caused by stomach acid — and to relieve GERD symptoms short-term (up to 8 weeks). In adults, they are also used to maintain that healing and reduce heartburn relapse, and to treat conditions where the stomach makes too much acid, including Zollinger-Ellison (ZE) Syndrome.

Intravenous (IV) pantoprazole — given by injection or infusion in a clinical setting — is approved in adults for short-term treatment of GERD with a history of erosive esophagitis (up to 10 days) and for pathological hypersecretory conditions including ZE Syndrome. PROTONIX I.V. is also approved for GERD/EE in pediatric patients 3 months and older (up to 7 days). IV pantoprazole is not approved for treating upper gastrointestinal bleeding.
