Why this applies to Itraconazole

Itraconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's the situation with moringa and itraconazole. Your body clears itraconazole using a liver enzyme called CYP3A4, and moringa might nudge that enzyme one way or the other. The catch is that lab studies disagree: some suggest moringa blocks this enzyme (which could let itraconazole build up, raising the odds of nausea, headache, or, rarely, liver or heart-rhythm trouble), while others suggest it speeds the enzyme up (which could lower itraconazole levels and let your fungal infection linger).

The good news: this is theoretical, based mostly on test-tube research, and one human study found no real change with a related drug. Everyday food amounts of moringa are unlikely to cause problems. Still, if you take concentrated moringa supplements, mention it to your pharmacist or doctor first.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist with moringa and your itraconazole (Sporanox, Tolsura). Itraconazole is moved around the body partly by a transporter called P-glycoprotein. In a lab dish, moringa leaf extract slowed that transporter down. If the same thing happened in people, your body might hold onto a little more itraconazole than usual, which could nudge up side effects like nausea, stomach upset, headache, or in rare cases effects on the liver or heart rhythm.

I want to be honest, though: this is only test-tube data, and it has not actually been seen in humans. So I wouldn't lose sleep over an occasional moringa tea or a sprinkle in your food. If you're taking concentrated moringa capsules daily, just mention it to your pharmacist or doctor so they can keep an eye on things.

About Itraconazole

What Is Itraconazole Used For?

Brand names: Sporanox, Sporanox Injection, Tolsura

Itraconazole capsules (including Sporanox capsules and generic itraconazole capsules) treat serious fungal infections — blastomycosis, histoplasmosis, and aspergillosis — in both people with normal immune systems and those who are immunocompromised. Capsule forms are also approved for nail fungus (onychomycosis) caused by dermatophytes in non-immunocompromised patients. Tolsura (a gelatin-coated capsule formulation) treats blastomycosis, histoplasmosis, and aspergillosis in adults, but is not approved for nail fungus and is not interchangeable with other itraconazole products.

Itraconazole oral solution is specifically approved for oropharyngeal candidiasis (thrush — a fungal infection of the mouth) and esophageal candidiasis (a fungal infection of the food pipe). Only the oral solution has been shown to work for these mouth and throat infections; capsules cannot be substituted for this use.
