Why this applies to Romidepsin

Romidepsin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's the situation with moringa and your romidepsin (Istodax). Your body uses an enzyme called CYP3A4 to help clear this chemotherapy drug. In lab studies, moringa has gone both ways: some research suggests it slows CYP3A4 down, and other research suggests it speeds it up. So we honestly can't say for sure which direction it would push your romidepsin levels.

If moringa slowed that enzyme, romidepsin could build up, meaning more nausea, fatigue, low blood counts, or heart rhythm changes. If it sped the enzyme up, the drug might clear too fast and work less well against the lymphoma. Keep in mind this is theoretical, from lab data only, and a human study with another CYP3A4 drug showed no change. Still, with chemo, please run any moringa supplement past your oncology team first.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the situation with moringa and your romidepsin (Istodax). Romidepsin is moved around the body partly by a transporter pump called P-glycoprotein. In lab dish studies, moringa leaf extract appears to slow that pump down. If that happened in a person, romidepsin might not get cleared as efficiently, so a little more could linger in your system. With a chemotherapy drug like this, that could in theory mean more of the usual side effects, things like nausea, fatigue, low blood counts, or changes on your heart tracing (EKG).

The honest part: this is based only on test-tube work, not real patients, so it's theoretical and unproven. Still, with cancer therapy I'd play it safe. Please check with your oncology team or pharmacist before adding a moringa supplement.

Romidepsin injection is approved to treat cutaneous T-cell lymphoma (CTCL) — a cancer that starts in white blood cells and primarily affects the skin — in adult patients who have already received at least one prior systemic (whole-body) therapy.

It is given intravenously (through a vein) in a clinical or infusion-center setting. It is not approved or recommended for peripheral T-cell lymphoma.
