CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with modafinil and moringa. Your liver uses an enzyme called CYP3A4 to help process modafinil. Moringa might nudge that enzyme one way or the other, but honestly the evidence is all over the place. Some lab tests suggest moringa slows the enzyme down (which could let modafinil build up a bit, possibly more headache, jitteriness, or trouble sleeping), while other lab work suggests it speeds the enzyme up (which could make modafinil work less well, so you feel sleepier).
Notice the word lab. This is test-tube and animal data, not real people on modafinil, and a human study with a related medicine showed no meaningful change. So I wouldn't lose sleep over it. Still, mention it to your pharmacist or doctor, especially if you start a concentrated moringa supplement, so they can keep an eye on how your medication is working.

In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist with moringa and your modafinil. In lab dishes, moringa leaf extract seems to slow down a cellular pump called P-glycoprotein, which helps move certain drugs out of the body. If that same effect happened in people, a little more modafinil could hang around, which in theory might nudge up its usual effects like jitteriness, headache, trouble sleeping, a faster heartbeat, or a slightly higher blood pressure.
The big caveat: this has only been seen in test-tube studies, never reported in actual people. So I wouldn't lose sleep over it, especially with food amounts. If you take concentrated moringa supplements regularly, it's worth a quick chat with your pharmacist or doctor so you can keep an eye on how you feel.

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).
In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
