CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Clarithromycin
Clarithromycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: your body clears clarithromycin using a liver enzyme called CYP3A4, and moringa has shown mixed signals in the lab. Some test-tube studies suggest moringa slows that enzyme down (which could let clarithromycin build up a bit, raising the odds of side effects like nausea, an upset stomach, diarrhea, or a strange metallic taste). Other studies suggest the opposite, that moringa speeds the enzyme up, which could weaken the antibiotic.
So the direction genuinely isn't settled. The good news is this is theoretical, based only on lab work, and a small human study with another drug found no real effect. Food amounts of moringa are unlikely to matter. Still, while you're on this antibiotic, check with your pharmacist before adding a concentrated moringa supplement.

What Is Clarithromycin Used For?
Brand names: Biaxin, Biaxin XL
Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.
Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
