Erdafitinib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the thing to notice about this pairing: the interaction on file runs in the opposite direction from what people usually worry about. It's not that erdafitinib (Balversa) changes evodia. Rather, drugs that block the liver enzyme CYP3A4 may slow down how your body clears evodia's active compounds, letting them build up. Erdafitinib does have some ability to inhibit CYP3A4, so in theory it could raise evodia levels and make its effects stronger.

This is based only on lab and animal studies, so it's theoretical and not proven in people. Still, erdafitinib is a serious cancer medicine with its own important side effects and many real drug interactions. I wouldn't add evodia without checking first. Please talk with your oncologist or pharmacist before combining them.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the short version: your bladder cancer medicine erdafitinib (Balversa) is cleared from your body partly by a liver enzyme called CYP3A4. Evodia, in lab studies, appears to slow that same enzyme down. If that happened in people, erdafitinib could build up to higher levels than expected, which may mean more of its typical side effects, things like dry mouth, mouth sores, changes in your nails or skin, diarrhea, and especially eye or vision problems and high phosphate levels that your oncology team watches closely.

That said, this is only a theoretical concern based on test-tube work, not anything shown in humans, so I don't want to alarm you. Since erdafitinib is a serious cancer drug with careful dosing, please run any Evodia use past your oncologist or pharmacist first rather than starting or stopping anything on your own.

About Erdafitinib

What Is Erdafitinib Used For?

Brand name: Balversa

Balversa (erdafitinib) is used to treat adults with locally advanced or metastatic urothelial carcinoma (UC) — cancer of the bladder or urinary tract — that has spread and has not responded to, or has gotten worse after, at least one prior round of systemic treatment. Patients must have a specific type of FGFR3 genetic alteration in their tumor, confirmed by an FDA-approved test before starting.

Balversa is not recommended for patients who are eligible for — but have not yet received — a PD-1 or PD-L1 inhibitor (a type of immunotherapy). Those therapies should be tried first when appropriate.
