Aprocitentan falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the twist with this one: the concern isn't that Aprocitentan (Tryvio) changes because of evodia. It's the other way around. Aprocitentan can mildly block the liver enzyme CYP3A4, and that same enzyme helps clear evodia's active compounds. So in theory, taking them together could let evodia build up a bit more in your system than usual.

I want to be honest about how thin the evidence is here. This is based on a lab and animal study using a much stronger blocker (ketoconazole), not real-world data in people taking Aprocitentan. So it's more of a caution than a red flag. Since evodia's own effects aren't well understood, it's smart to run this combo past your pharmacist or doctor before mixing them.

Here's the direction that matters with this pairing: Aprocitentan (Tryvio) can act as a CYP3A4 inducer, meaning it can speed up the liver enzyme that helps clear certain compounds. Evodia's active ingredients (like evodiamine and rutaecarpine) are broken down by that same pathway. So the concern isn't that Evodia changes your blood pressure medicine, it's the other way around: Aprocitentan may cause your body to clear Evodia faster, leaving less of the supplement around to do whatever you were hoping it would do.

Honestly, this is based only on lab and animal studies, so it's more theoretical than proven, and it doesn't suggest harm to your blood pressure treatment. Still, since Aprocitentan is a newer medication, please run any supplement past your pharmacist or doctor first.

What Is Aprocitentan Used For?

Brand name: Tryvio

Tryvio is used to treat high blood pressure (hypertension) in adults. It is specifically for people whose blood pressure is not adequately controlled on other blood pressure medications — it is always used together with at least one other antihypertensive drug, not on its own.

Lowering blood pressure helps reduce the risk of serious cardiovascular events such as stroke and heart attack. Tryvio has not been studied as a standalone treatment and is not intended to replace other blood pressure medicines.
