Erdafitinib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the connection: your body turns adrafinil into modafinil, and modafinil is cleared mostly by a liver enzyme called CYP3A4. Erdafitinib (Balversa) can mildly block that same enzyme. So in theory, taking the two together could slow the breakdown of modafinil and let it build up a bit, which may mean more wakefulness-promoting effects like trouble sleeping, jitteriness, a racing heart, headache, or higher blood pressure.

I want to be honest about how strong this is: it's a theoretical concern based on how the drugs work, not something proven in studies, and it's rated moderate. Bigger picture, adrafinil itself is unregulated and can stress the liver. Since you're on cancer treatment, please loop in your oncology team or pharmacist before mixing anything new.

Here's the gist for Erdafitinib (Balversa). Your body turns adrafinil into modafinil, and lab studies suggest modafinil can lower the activity of a liver enzyme called CYP2C9, which helps clear erdafitinib. If that enzyme slows down, erdafitinib could build up a bit more than expected, which may mean stronger effects and more side effects.

With erdafitinib, that could look like worse mouth sores, dry eyes or vision changes, high phosphate levels, diarrhea, dry skin, or nail problems. I'll be honest though, this concern is theoretical and based only on lab data, not real patient reports. Since erdafitinib is a serious cancer medicine, please don't change anything on your own. Run any supplement past your oncology team or pharmacist first.

Here's the concern when you pair these two. Once your liver converts adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4. Your bladder cancer medicine, erdafitinib (Balversa), is partly broken down by that same enzyme. So in theory, revving it up could clear erdafitinib out a little faster and leave you with lower levels of the drug.

Why that matters: erdafitinib is doing serious work fighting your cancer, and you really don't want anything weakening it. The good news is this interaction is only theoretical, based on how the drugs behave, not on strong real-world data. Still, with a cancer medicine I'd play it safe. Please run any adrafinil use past your oncologist or pharmacist first.

About Erdafitinib

What Is Erdafitinib Used For?

Brand name: Balversa

Balversa (erdafitinib) is used to treat adults with locally advanced or metastatic urothelial carcinoma (UC) — cancer of the bladder or urinary tract — that has spread and has not responded to, or has gotten worse after, at least one prior round of systemic treatment. Patients must have a specific type of FGFR3 genetic alteration in their tumor, confirmed by an FDA-approved test before starting.

Balversa is not recommended for patients who are eligible for — but have not yet received — a PD-1 or PD-L1 inhibitor (a type of immunotherapy). Those therapies should be tried first when appropriate.
