Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
I want to be upfront with you here: the interaction on file was really written for true immunosuppressant medicines, the kind that calm down an overactive immune system. Lumateperone (Caplyta) is not one of those. It is an atypical antipsychotic that works on brain chemicals like dopamine and serotonin to help with schizophrenia symptoms. It does not touch your immune system, so the whole idea behind this warning (that fermented wheat germ extract might rev up immune activity and fight against an immunosuppressant) simply does not apply to Caplyta.
So in practical terms, I would not expect this supplement to change how well your Caplyta works or cause new side effects through this pathway. That said, fermented wheat germ extract is still mostly studied in cancer care with limited evidence, so please run it by your doctor or pharmacist before starting it.

Caplyta (lumateperone) is approved for adults with schizophrenia, depressive episodes associated with bipolar I or II disorder (bipolar depression), and as an add-on treatment for major depressive disorder (MDD).
For bipolar depression, Caplyta can be used on its own or alongside lithium or valproate. For MDD, Caplyta is used together with an antidepressant — not on its own as a standalone antidepressant.
