Brompheniramine, Dextromethorphan, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the gist with your Dimetapp DM. Two of its ingredients, the antihistamine brompheniramine and the cough suppressant dextromethorphan, are partly cleared by a liver enzyme called CYP2D6. Fever bark contains a compound (yohimbine) that, in lab dishes, can slow that enzyme down. If that happened in real life, these ingredients might hang around longer and hit harder, meaning more drowsiness, dry mouth, dizziness, or with dextromethorphan, feeling foggy or jittery.

The honest truth: this is only theoretical, based on test-tube work, and fever bark's actual effect in people isn't known. I'd still steer clear of fever bark in general, since its alkaloids can affect blood pressure and heart rate on their own. Please run it by your pharmacist or doctor before combining them.

Reserpine, a constituent of fever bark, might increase or decrease effects of sympathomimetics. Also, yohimbine, another constituent of fever bark, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. It is not known if this would occur with fever bark.

Here's the piece that matters most in your Dimetapp DM: the phenylephrine. It's a decongestant that works by tightening blood vessels, which can nudge blood pressure and heart rate up. Fever bark contains plant alkaloids that also tug on the same system. One of them, a yohimbine-like compound, can raise blood pressure on its own, while a reserpine-like compound can pull the other way. Because they push in opposite directions, the combined effect is genuinely hard to predict.

In practice, you might notice a faster heartbeat, jitteriness, headache, or blood pressure swings. To be honest, this is theoretical and rests on weak, anecdotal evidence, so I wouldn't panic. Still, I'd skip fever bark and check with your pharmacist or doctor first, especially if you have heart or blood pressure concerns.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
