Codeine Phosphate, Pseudoephedrine, Triprolidine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's a wrinkle worth knowing about this combo cough and cold product. The codeine part is actually a prodrug, meaning your body has to switch it on using a liver enzyme called CYP2D6 to turn it into morphine, which is what gives the pain and cough relief. Fever bark contains a yohimbine-like compound that may block that very enzyme. If it does, less codeine gets activated, so you could get weaker cough and pain relief than expected.

Honestly, this is theoretical and based only on lab studies, not real people, so I wouldn't lose sleep over it. But fever bark's own alkaloids can stir up blood pressure and heart rate, which doesn't mix well with the pseudoephedrine here. Please run this past your pharmacist or doctor before combining them.

Theoretically, fever bark might increase or decrease the effects of stimulant drugs.
Reserpine, a constituent of fever bark, might increase or decrease effects of sympathomimetics. Also, yohimbine, another constituent of fever bark, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. It is not known if this would occur with fever bark.

The piece of this combo that matters most here is the pseudoephedrine, which is a decongestant that works by stimulating your system, raising blood pressure and heart rate a bit. Fever bark contains plant alkaloids (including yohimbine-like and reserpine-like compounds) that can also push on blood pressure and the nervous system. Stacking the two could either add up to more stimulation (think jitters, a racing or pounding heart, higher blood pressure, trouble sleeping) or pull in opposite directions, so the effect is honestly unpredictable.

I want to be straight with you: this is a theoretical concern based mostly on anecdotal reports, not solid human studies on fever bark itself. Still, since fever bark has little proven benefit and some potent ingredients, I'd run this past your pharmacist or doctor before combining them rather than guess.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
