Goji and Aspirin, Omeprazole

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Goji interacts with whole categories of medication. Aspirin, Omeprazole falls into 3 of those categories, so it can interact with Goji in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 3

Why this applies to Aspirin, Omeprazole

Aspirin, Omeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2C19 and reduce metabolism of CYP2C19 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the piece that matters for Yosprala: the omeprazole part is cleared by a liver enzyme called CYP2C19. In lab (test-tube) studies, goji appears to slow that enzyme down. If that held true in people, omeprazole could build up a bit and have a slightly stronger stomach-acid-blocking effect. The aspirin portion isn't really cleared this way, so it's mostly omeprazole we're thinking about here.

Honestly, this is a theoretical concern. It has only been seen in the lab, never reported in actual patients, and eating goji berries as food is very unlikely to cause a problem. Concentrated goji tinctures or juices are where I'd be a little more cautious. If you use those regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 3

Aspirin, Omeprazole also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, goji berry might inhibit CYP2C9 and reduce metabolism of CYP2C9 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.

Here's the gist with Yosprala, which pairs aspirin with omeprazole. Goji berries may block a liver enzyme called CYP2C9, the same pathway that helps clear part of this medicine. In theory, that could let levels build up a bit. For omeprazole, higher levels usually just mean stronger acid suppression. For aspirin, more in your system could nudge up the risk of stomach upset, heartburn, or bleeding, like easy bruising or a longer time to stop bleeding from a small cut.

The honest truth: this is mostly lab and case-report evidence, mostly tied to warfarin, so it's far from certain. Eating goji as a food is unlikely to matter. Concentrated goji juice, tea, or capsules are the bigger question. If you use those regularly, give your pharmacist or doctor a heads-up first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Aspirin, Omeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the gist with Yosprala (the aspirin plus omeprazole combo) and goji. Lab studies suggest goji might slow down a liver enzyme called CYP3A4. Of the two ingredients here, omeprazole is partly handled by that pathway, so in theory more of it could hang around. In practice that might mean slightly stronger acid-blocking, but omeprazole is generally well tolerated, and this hasn't actually been seen in people. The aspirin part isn't really cleared by this enzyme, so I wouldn't expect goji to change your stroke or heart protection.

Honestly, this is test-tube level evidence only, and eating a normal serving of goji berries is unlikely to matter. Concentrated goji supplements are a different story, so if you plan to use one, just run it by your pharmacist or doctor first.

From our pharmacist’s overview on the Aspirin, Omeprazole page

“Yosprala pairs aspirin, a blood thinner, with omeprazole, an acid reducer whose job is to shield the stomach from that aspirin, so this list carries real weight and warrants genuine care. Of the 318 supplements and herbs on file, nine are rated major, and understanding those first matters more than reading the rest.”
