Why this applies to Belzutifan

Belzutifan falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2C19 and reduce metabolism of CYP2C19 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.

Here's the gist for Welireg (belzutifan): your liver clears this medication partly through an enzyme called CYP2C19. In lab tests (not in actual people), goji berry seems to slow that same enzyme down. If goji blocks it in real life, belzutifan could clear out a little slower and build up in your blood, which might mean stronger effects and more side effects like anemia (feeling tired, short of breath, or pale) and low oxygen levels in your tissues.

I want to be honest though: this is only a theoretical, lab-based concern, and it has never actually been seen in humans. A few goji berries as food are very unlikely to matter. Still, because belzutifan is a serious cancer drug, please check with your oncologist or pharmacist before using concentrated goji supplements.

About Belzutifan

What Is Belzutifan Used For?

Brand name: Welireg

Welireg (belzutifan) is approved for several serious conditions. In adults with von Hippel-Lindau (VHL) disease, it treats associated kidney cancer (renal cell carcinoma), brain and spine tumors called CNS hemangioblastomas, and pancreatic neuroendocrine tumors — when surgery is not immediately needed.

Welireg also treats adults with clear cell renal cell carcinoma (ccRCC): as an adjuvant treatment (after surgery to remove the kidney) in combination with pembrolizumab or pembrolizumab and berahyaluronidase alfa-pmph to reduce the risk of recurrence, and as a single agent for advanced ccRCC after certain prior therapies have been tried. Additionally, Welireg treats adults and patients aged 12 and older with locally advanced, unresectable, or metastatic pheochromocytoma or paraganglioma (PPGL) — rare tumors of the adrenal glands or nearby nerve tissue.
