Benzhydrocodone, Acetaminophen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2D6 and reduce metabolism of CYP2D6 substrates.
In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.

Here's the wrinkle with Apadaz: the benzhydrocodone part is a prodrug. Your body has to convert it into hydrocodone, and then an enzyme called CYP2D6 turns some of that into hydromorphone, which adds to the pain relief. Lab studies suggest goji might block CYP2D6.

Because this is a prodrug, blocking that enzyme could actually lessen some of the pain-relieving punch, though levels of hydrocodone itself may also linger. Honestly, this is only seen in test-tube research, not in real people, so I wouldn't lose sleep over a few goji berries in your oatmeal. With concentrated goji supplements it's worth a quick chat with your pharmacist. If your pain isn't well controlled, tell your doctor.

Here's the thing to know about Apadaz: the benzhydrocodone part is a prodrug, meaning your body has to convert it into hydrocodone before it relieves pain. Lab studies hint that goji berry can slow down CYP3A4, one of the enzymes that helps process this medicine. If that happened in real life, hydrocodone could build up a bit, which might mean stronger drowsiness, dizziness, constipation, nausea, or slowed breathing.

I'll be honest with you though, this is only a test-tube finding and has never actually been reported in people. Eating goji berries in normal food amounts is very unlikely to cause trouble. Concentrated goji supplements or juice are the bigger question mark. If you want to use those alongside Apadaz, just run it by your pharmacist or doctor first.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
