Cabozantinib S-Malate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the gist for your cancer medicine Cabometyx (cabozantinib). Your body clears this drug using a liver enzyme called CYP3A4. In lab studies, goji berry juice appears to slow that same enzyme down. If that happened in real life, less of the drug would be broken down, so more could build up in your body. That could mean stronger side effects, things like diarrhea, mouth sores, high blood pressure, fatigue, hand-foot skin reactions, or trouble with wound healing.

The important part: this is only seen in test tubes, not in people, so it's theoretical. Still, with a serious cancer drug like this, I wouldn't gamble on concentrated goji supplements. Eating a few goji berries as food is far less concerning. Please check with your oncology team or pharmacist before adding any goji product.

About Cabozantinib

What Is Cabozantinib Used For?

Brand name: Cabometyx

Cabozantinib treats several types of cancer. Cabometyx (tablet) is used for advanced renal cell carcinoma (kidney cancer) — either alone or combined with nivolumab as a first-line treatment — as well as hepatocellular carcinoma (liver cancer) in patients previously treated with sorafenib, differentiated thyroid cancer that has progressed after prior therapy, and pancreatic or extra-pancreatic neuroendocrine tumors (NETs) that have been previously treated. For thyroid cancer and NETs, Cabometyx is approved for adults and adolescents aged 12 and older.

Cometriq (capsule) is approved specifically for progressive, metastatic medullary thyroid cancer (a different type of thyroid cancer). Cometriq and Cabometyx are not interchangeable — do not substitute one for the other.
