These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Goji interacts with whole categories of medication. Dextromethorphan HBr falls into 2 of those categories, so it can interact with Goji in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Dextromethorphan HBr
Dextromethorphan HBr falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with goji and your cough medicine. Dextromethorphan is cleared from your body mainly by a liver enzyme called CYP2D6. Goji berry, at least in lab dish studies, seems to slow that enzyme down. If that happened in real life, your body would break down the dextromethorphan a little slower, so more of it could hang around. That might mean stronger or longer effects, like drowsiness, dizziness, or feeling foggy.
Now, the honest part: this is only seen in test tube research, not in actual people, so it may not matter at all, especially with normal food amounts of goji. Concentrated goji supplements are the bigger question mark. If you use those regularly, just give your pharmacist or doctor a quick heads-up.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Dextromethorphan HBr also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your cough medicine. One of the liver enzymes that helps clear dextromethorphan is called CYP3A4. In lab dish studies, goji berry juice seemed to slow this enzyme down. If that happened in your body, dextromethorphan could hang around a bit longer than usual, which in theory might mean more drowsiness, dizziness, or that foggy, lightheaded feeling some people get from cough syrups.
The big caveat: this has only been seen in test tubes, never actually reported in real people, so I wouldn't lose sleep over it. Eating goji berries as a food is very unlikely to matter. If you're using concentrated goji supplements alongside cough medicine, just mention it to your pharmacist or doctor to be safe.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.

- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
