In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.

Here's the wrinkle with this combo. The hydrocodone part of your cough medicine is a prodrug. Your body uses an enzyme called CYP2D6 to turn some of it into hydromorphone, which is a stronger painkiller. Lab studies hint that goji berry might slow down CYP2D6. If that happened in people, you'd actually convert less hydrocodone into the stronger form, so you might get a bit less pain or cough relief, not more.

The honest truth is this is only test-tube evidence, never shown in real patients, so I wouldn't lose sleep over it. A handful of goji berries in food is very unlikely to matter. If you're using concentrated goji supplements, just run it by your pharmacist or doctor first.

Here's the situation with your cough syrup that pairs guaifenesin with hydrocodone. The hydrocodone part leans on a liver enzyme called CYP3A4 to be broken down. In lab dishes, goji berry juice seems to slow that same enzyme. If that played out in real life, your body might clear the hydrocodone a little slower, letting it build up. With an opioid, that could mean more drowsiness, dizziness, constipation, or in a worst case, slower or shallower breathing.

The honest part: this is only seen in test-tube studies, never reported in actual people, so I wouldn't lose sleep over a few goji berries in your oatmeal. Concentrated goji extracts are a different story. Just give your pharmacist or doctor a heads up before mixing them.
