Why this applies to Idelalisib

Idelalisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here is the short version: your body relies on a liver enzyme called CYP3A4 to break down idelalisib and keep its level in a safe range. Lab studies suggest goji berry juice can slow that same enzyme down. In theory, that could let idelalisib build up a bit higher than expected, which might mean a stronger chance of its known side effects, things like diarrhea, liver enzyme bumps, low blood counts, or lung inflammation.

I want to be honest about the strength of this, though. So far this is only seen in test-tube research, not in real people, so we don't actually know if it matters in everyday life. Idelalisib is a serious cancer medicine with careful monitoring, so before adding concentrated goji supplements, please run it by your oncology team or pharmacist first.

What Is Idelalisib Used For?

Brand name: Zydelig

Zydelig (idelalisib) is approved to treat relapsed chronic lymphocytic leukemia (CLL) — a cancer of the blood and bone marrow — when used together with rituximab. It is specifically for patients who have already received treatment and for whom rituximab alone would be appropriate due to other health conditions.

Zydelig is not approved or recommended as a first-line (initial) treatment for CLL or any other cancer, including small lymphocytic lymphoma (SLL) or follicular lymphoma (FL). It is also not approved in combination with bendamustine and rituximab, or with rituximab alone, for FL, SLL, or other slow-growing non-Hodgkin lymphomas.
