Why this applies to Ivacaftor, Lumacaftor
Ivacaftor, Lumacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Orkambi (lumacaftor/ivacaftor) and goji. The ivacaftor part of this combo is handled mainly by a liver enzyme called CYP3A4. Goji berry, at least in test-tube studies, appears to slow that enzyme down a bit. In theory, that could let ivacaftor build up a little more than usual, which might mean a higher chance of side effects like headache, dizziness, belly upset, or changes in liver-related blood tests.
The important thing to keep in mind: this is only seen in lab research and has never actually been reported in people, so the real-world risk is likely small. A few goji berries as food are very unlikely to matter, while concentrated goji supplements or juice are a bigger question mark. Since cystic fibrosis treatment really matters, just run any goji supplement past your CF team or pharmacist first.

What Is Lumacaftor and Ivacaftor Used For?
Brand name: Orkambi
Lumacaftor and ivacaftor (Orkambi) treats cystic fibrosis (CF) in people aged 1 year and older who are homozygous for the F508del mutation in the CFTR gene. Homozygous means you inherited the mutation from both parents.
If your genetic makeup is unknown, an FDA-cleared CF mutation test is used to check for the mutation on both copies of the gene. Orkambi has not been shown to be safe or effective in people with CF who do not have two copies of F508del.
