Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the situation with goji and Lorbrena (lorlatinib). Your body relies on a liver enzyme called CYP3A4 to break down lorlatinib and keep its level steady. Lab studies hint that goji berry juice may slow that enzyme down. If that happened in real life, lorlatinib could clear more slowly and build up, which could mean more of its known side effects, things like high cholesterol or triglycerides, swelling, weight gain, fatigue, mood or memory changes, or nerve tingling.

The honest part: this is only a test-tube finding and has never been confirmed in people, so the risk is theoretical. A few berries in food are very unlikely to matter, but concentrated goji supplements or juice are a different story. Since lorlatinib is a serious cancer drug, please run any goji product by your oncologist or pharmacist first.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
