Pioglitazone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with goji and your pioglitazone (Actos). Your body partly clears pioglitazone using a liver enzyme called CYP3A4. In lab dish studies, goji berry juice appears to slow that enzyme down. If that happened in real life, pioglitazone could hang around longer and act a little stronger, which might mean things like swelling in the legs or ankles, weight gain, or a bigger drop in blood sugar if you take it with other diabetes meds.
I want to be honest about the evidence though: this is only test-tube data and has not been shown in actual people. Eating goji berries as food is unlikely to matter much. Concentrated goji supplements are the bigger question mark, so just run those by your pharmacist or doctor first.

ANTIDIABETES DRUGS

Animal and in vitro research show that goji root bark and fruit polysaccharides might have hypoglycemic effects. However, clinical research has only shown that taking goji fruit polysaccharides with or without antidiabetes drugs modestly reduces postprandial glucose when compared with control, with no reports of hypoglycemia.

Here's the simple version: goji isn't changing how your body breaks down pioglitazone. Instead, goji (especially concentrated polysaccharide supplements) may have a mild blood-sugar-lowering effect of its own. Stacked on top of pioglitazone, that could add up and push your sugar a little lower than expected.
Honestly, this is a minor concern. The human studies only showed a modest dip in after-meal glucose, and no one reported true low blood sugar. Pioglitazone by itself rarely causes lows unless you also take insulin or a sulfonylurea. Still, if you start a goji supplement, just keep an eye out for shakiness, sweating, or lightheadedness, and check your readings. Eating a few goji berries as food is very unlikely to matter. Loop in your pharmacist or doctor before adding any concentrated supplement.

Pioglitazone tablets — sold as Actos and under the generic names Pioglitazone and Pioglitazone Hydrochloride — are used to improve blood sugar (glucose) control in adults with type 2 diabetes. They are taken along with diet and exercise, either alone or combined with other diabetes medicines such as a sulfonylurea, metformin, or insulin.
Pioglitazone is not for type 1 diabetes or diabetic ketoacidosis (a serious complication where the blood becomes too acidic). It only works when the body is still producing some of its own insulin.
