Goji and Venetoclax

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Venetoclax

Venetoclax falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with goji and Venclexta (venetoclax). Your body clears venetoclax mainly through a liver enzyme called CYP3A4. Lab studies hint that goji berry juice may slow that enzyme down. If that happened in a person, venetoclax could build up to higher levels than expected, which can mean more side effects like low blood counts, infections, nausea, diarrhea, or tiredness. With venetoclax there's also a serious early concern called tumor lysis syndrome, so keeping its levels steady really matters.

The important caveat: this is only test-tube data and has not been seen in actual patients, so the real risk is uncertain. Still, since venetoclax is a high-stakes cancer drug with strict CYP3A4 rules, please check with your oncology team or pharmacist before using concentrated goji supplements.

From our pharmacist’s overview on the Venetoclax page

“Venetoclax (Venclexta) is a targeted leukemia and lymphoma drug whose blood level is managed carefully, since too much invites serious side effects and too little gives the cancer room. That makes this a list to treat with genuine caution.”
