Here's the deal with your birth control pill (the ethinyl estradiol and norgestimate combo) and green tea. Both are processed by your liver. On their own, the estrogen in birth control pills can occasionally stress the liver, and concentrated green tea extract supplements have been tied to rare cases of liver irritation. The worry is that taking both together could, in theory, add up and put a little extra strain on your liver.

I want to be honest: this is based on scattered anecdotal reports, not strong proof, and it's considered unlikely to happen. Drinking green tea as a beverage almost certainly isn't a concern. The caution is really about high-dose extract pills. If you ever notice yellowing skin, dark urine, or belly pain, check in with your doctor or pharmacist.

CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

2 of 6

Ethinyl Estradiol, Norgestimate also falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Green tea contains caffeine. Caffeine is metabolized by cytochrome P450 1A2 (CYP1A2),. Theoretically, drugs that inhibit CYP1A2 may decrease the clearance rate of caffeine from green tea and increase caffeine levels.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the twist with this pairing: it's actually your birth control pill, Ortho Tri-Cyclen and similar brands, that may slow down how your body clears the caffeine in green tea, not the other way around. The estrogen in these pills can mildly block the CYP1A2 enzyme that breaks caffeine down. So the caffeine from your green tea (or your coffee, for that matter) might hang around a bit longer and hit a little harder.

In real life that could mean feeling more jittery, a racing heart, trouble sleeping, or an upset stomach. This is a theoretical interaction based on how the drug works, not strong studies, so for most people it's mild. A regular cup or two of green tea is unlikely to be a problem. If you notice you feel wired, just ease back on caffeine and chat with your pharmacist.

Ethinyl Estradiol, Norgestimate also falls under ESTROGENS.

Theoretically, estrogens might increase the levels and adverse effects of caffeine.
Green tea contains caffeine. Estrogen inhibits caffeine metabolism.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's a twist that often surprises people: this interaction isn't really about your birth control pill changing. It's about the caffeine in green tea. The estrogen in Ortho Tri-Cyclen (ethinyl estradiol) can slow down how your liver breaks caffeine down, so caffeine tends to hang around longer in your body.

In everyday terms, that cup of green tea may hit you a bit harder than usual. You might notice things like feeling jittery, a racing heart, trouble sleeping, or an upset stomach. Your pill keeps working normally and stays just as effective. This is mostly a comfort issue, not a danger. If green tea or coffee leaves you wired, simply cutting back usually helps. Check with your pharmacist if you're unsure.

P-GLYCOPROTEIN SUBSTRATES

4 of 6

Ethinyl Estradiol, Norgestimate also falls under P-GLYCOPROTEIN SUBSTRATES.

Green tea might increase the levels and adverse effects of P-glycoprotein (P-gp) substrates.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your birth control pill (Ortho Tri-Cyclen and similar brands). One of the proteins your body uses to pump drugs back out, called P-glycoprotein, helps move the estrogen part of your pill. The thinking is that green tea may slow that pump down a little, which could let slightly more of the hormone stay in your system. In theory that might mean a bit more of the usual estrogen-type side effects, like nausea, breast tenderness, headaches, or bloating.

I want to be honest about how shaky this evidence is. It comes mostly from lab studies and a few scattered case reports, so it's far from proven. A normal cup or two of green tea is very unlikely to matter. Concentrated green tea extract pills are where I'd be a little more cautious. If you take those, just run it by your pharmacist or doctor first.

CONTRACEPTIVE DRUGS

5 of 6

Ethinyl Estradiol, Norgestimate also falls under CONTRACEPTIVE DRUGS.

Theoretically, concomitant use might increase the effects and adverse effects of caffeine found in green tea.
Green tea contains caffeine. Oral contraceptives can decrease caffeine clearance by 40% to 65%.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's something a little different about this pairing. It's not that green tea changes how your birth control (Ortho Tri-Cyclen and similar) works, it's actually the other way around. The estrogen in your pill can slow down how your body clears caffeine, sometimes by 40% to 65%. Green tea naturally contains caffeine, so it may stick around longer and hit you harder than usual.

In real terms, that means a few cups of green tea might leave you feeling jittery, anxious, restless, or having trouble sleeping, or give you a faster heartbeat. This is more likely with concentrated green tea extract supplements than with a normal cup or two. If you notice these effects, simply easing back on caffeine usually helps. Chat with your pharmacist or doctor if you're unsure.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

6 of 6

Ethinyl Estradiol, Norgestimate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Good news here: if you enjoy green tea while taking your birth control pill (the ethinyl estradiol and norgestimate combo found in brands like Sprintec or Ortho Tri-Cyclen), you don't need to worry. Your pill is processed by a liver enzyme called CYP3A4, and lab studies hint that green tea might nudge that enzyme one way or the other. But in actual people, green tea hasn't been shown to meaningfully change how this medication works.

In plain terms, drinking your usual cups of green tea is very unlikely to weaken your contraceptive protection or cause extra estrogen-type side effects like nausea or breast tenderness. The evidence is weak and mostly from test tubes. If you're considering a concentrated green tea extract rather than the beverage, just run it by your pharmacist or doctor first.

About Green Tea

What is Green Tea?

Green Tea
  • Antioxidant support
  • Weight loss
  • Heart health
  • Mental alertness
  • Cancer risk reduction

Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.
