Green Tea and Finafloxacin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

QUINOLONE ANTIBIOTICS

Why this applies to Finafloxacin

Finafloxacin falls under QUINOLONE ANTIBIOTICS — the group Green Tea interacts with.

Theoretically, quinolone antibiotics might increase the levels and adverse effects of caffeine.
Green tea contains caffeine. Quinolones (also called fluoroquinolones) can decrease caffeine clearance by inhibiting cytochrome P450 1A2 (CYP1A2) enzyme.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the twist with this pairing: it's not the antibiotic that builds up, it's the caffeine from your green tea. Finafloxacin (Xtoro) belongs to the quinolone family, and these drugs can slow down the liver enzyme (CYP1A2) that normally clears caffeine. With that cleanup slowed, caffeine can linger longer and hit harder, leaving you feeling jittery, restless, with a racing heart or trouble sleeping.

The good news? Xtoro is actually ear drops used for swimmer's ear, so very little gets into your bloodstream. That makes this mostly theoretical for the drop form. A cup or two of green tea is unlikely to cause trouble, but if you're loading up on tea or caffeine pills, mention it to your pharmacist.

From our pharmacist’s overview on the Finafloxacin page

“The clearest snag on the supplement list for finafloxacin (Xtoro), a quinolone antibiotic ear drop for outer ear infections, is minerals. Calcium and iron can bind quinolone drugs in the gut and cut how much gets absorbed, so those doses belong hours apart.”

About Green Tea

What is Green Tea?

Green Tea
  • Antioxidant support
  • Weight loss
  • Heart health
  • Mental alertness
  • Cancer risk reduction

Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.
