Why this applies to Irinotecan Hydrochloride

Irinotecan Hydrochloride falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Green Tea interacts with.

Theoretically, green tea might reduce the absorption of organic anion-transporting polypeptide (OATP) substrates.
OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds. Research shows that two of the major catechins found in green tea, epicatechin gallate (ECG) and epigallocatechin gallate (EGCG), inhibit OATPs, specifically OATP1A2, OATP1B1, and OATP2B1. In addition, green tea has been shown to reduce the absorption of some drugs that are OATP substrates, including lisinopril, and celiprolol.

Here's the idea behind this one. Green tea, especially in concentrated extract form, contains catechins (ECG and EGCG) that can block transporters called OATPs. These transporters help move certain medicines into cells, so blocking them could lower how much of a drug your body picks up. With irinotecan, that could theoretically mean less of the medicine reaching where it needs to work, which matters a lot for a cancer treatment you don't want weakened.

That said, this is based only on lab and animal studies, not real-world results with irinotecan, so it stays fairly theoretical. A cup or two of green tea is unlikely to be the issue; high-dose extract pills are the bigger question. Since you're on chemotherapy, please run any green tea supplement past your oncology team before starting it.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Here's the short version: drinking green tea is very unlikely to cause a meaningful problem with your irinotecan (Onivyde). Irinotecan does get processed in part by a liver enzyme called CYP3A4, and lab and animal studies hint that green tea can nudge this enzyme in either direction. But in actual people, green tea doesn't seem to change CYP3A4 activity enough to matter, so the everyday cup you enjoy shouldn't shift how your chemo works or how harsh its side effects (like diarrhea, low blood counts, or nausea) feel.

That said, irinotecan is a serious chemo drug, so I'd still mention any concentrated green tea extract or supplement to your oncology team before starting it. They can give you the all-clear for your specific situation.

  • Irinotecan lipid complex is used in combination with other medications to treat pancreatic cancer that has spread to other parts of the body that has worsened after treatment with other chemotherapy medications.
