P-GLYCOPROTEIN SUBSTRATES

Riociguat falls under P-GLYCOPROTEIN SUBSTRATES — the group Green Tea interacts with.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with riociguat (Adempas): your body uses a kind of pump called P-glycoprotein to help move this drug along. Green tea may slow that pump down a little, which could let slightly more riociguat hang around in your blood. Since riociguat relaxes blood vessels, more of it could mean too much blood-pressure lowering, with things like dizziness, lightheadedness, headache, an upset stomach, or feeling faint when you stand up.
I want to be honest about the evidence here: it's pretty thin, mostly lab studies and a handful of case reports, so this is a possible concern rather than a sure thing. A cup or two of regular green tea is far less likely to matter than concentrated green tea extract pills. Don't change anything on your own. Just give your pharmacist or doctor a heads-up so they can keep an eye on your symptoms.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Riociguat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the good news on this one: Riociguat (Adempas) is handled by a liver enzyme called CYP3A4, and green tea has only a faint, theoretical effect on it. Lab and animal studies hint that green tea might nudge this enzyme either way, but in actual people it doesn't seem to change CYP3A4 much at all. So drinking your usual cup or two of green tea is very unlikely to make a real difference in how riociguat works.
In theory, if green tea slowed riociguat's breakdown, levels could rise and you might notice more low blood pressure effects like dizziness, lightheadedness, or headache. But this is unlikely. Concentrated green tea extract pills are stronger than the beverage, so if you're considering those, check with your pharmacist or doctor first.

About Riociguat
What Is Riociguat Used For?
Brand name: Adempas
Riociguat tablets — sold under the brand names Adempas and Riociguat — are approved for adults with two types of high blood pressure in the lungs. The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots that have blocked or narrowed the arteries in the lungs. Riociguat is used when surgery has not fully fixed the problem, or when surgery is not an option.
The second is pulmonary arterial hypertension (PAH), a progressive disease where the arteries in the lungs become stiff and narrowed. In PAH, riociguat helps improve exercise ability, functional class (how well you can get around day-to-day), and slows disease worsening. It can be used alone or combined with other PAH medicines called endothelin receptor antagonists or prostanoids.
