P-GLYCOPROTEIN SUBSTRATES

Telaprevir falls under P-GLYCOPROTEIN SUBSTRATES — the group Green Tea interacts with.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for telaprevir (Incivek): your body uses a little cellular pump called P-glycoprotein to help move this drug along and clear some of it out. Green tea may gently slow that pump down. In theory, that could let a bit more telaprevir hang around in your bloodstream, which might mean more of the usual side effects, things like itchy rash, nausea, an upset stomach, fatigue, or anemia (especially since it's taken with ribavirin and peginterferon).
I want to be straight with you, though: this is based on lab studies and a handful of scattered case reports, so it's weak evidence and not a sure thing. Everyday cups of green tea are far less concentrated than green tea extract pills. Still, before you add any green tea supplement, check with your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Telaprevir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: your liver and gut handle telaprevir using an enzyme called CYP3A4, and green tea can nudge that enzyme in a couple of directions in lab tests. In a test tube, green tea seems to block CYP3A4 in the gut while speeding it up in the liver. On paper that's a mixed signal, but in real people green tea doesn't appear to meaningfully change how this enzyme works.
So practically, normal cups of green tea are very unlikely to change your telaprevir levels or cause more side effects like nausea, rash, or itching. This concern is mostly theoretical and comes from lab data, not human studies. Still, since telaprevir is part of a serious hepatitis C regimen, and concentrated green tea extracts can stress the liver, it's worth a quick check with your pharmacist or doctor before adding any supplement form.

Telaprevir is used along with two other medications (ribavirin [Copegus, Rebetol] and peginterferon alfa [Pegasys]) to treat chronic hepatitis C (an ongoing viral infection that damages the liver) in people who have not yet been treated for this condition or whose condition could not successfully be treated with ribavirin and peginterferon alfa alone. Telaprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Telaprevir may not prevent the spread of hepatitis C to other people.
