Why this applies to Brexpiprazole

Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the gist with these two together. Your body relies partly on an enzyme called CYP3A4 to clear brexpiprazole (Rexulti) out of your system. The thinking is that gymnema might slow that enzyme down a little, which in theory could let brexpiprazole build up and nudge up side effects like drowsiness, restlessness, dizziness, or weight gain.

But I want to keep this in perspective. This concern comes only from a single lab test using rat liver cells. Other lab and animal studies found gymnema didn't change CYP3A4 at all. So this is very much a theoretical, low-likelihood interaction, not something proven in people. Still, since brexpiprazole is a serious medication, it's worth a quick mention to your pharmacist or doctor before adding gymnema.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
