Why this applies to Buproprion, Dextromethorphan

Buproprion, Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Auvelity is a combo of bupropion and dextromethorphan, used for depression. The dextromethorphan piece is partly handled by the CYP3A4 enzyme in your liver, and the worry on file is that gymnema could slow that enzyme a little, letting levels creep up. In theory that might mean more dizziness, drowsiness, nausea, or a higher chance of side effects from the dextromethorphan part.

Honestly though, this is a very weak signal. It comes only from a rat liver test tube study, while human liver studies and live animal studies showed no real effect on CYP3A4. So this is largely theoretical and unlikely to cause trouble. Still, since both gymnema and this medicine can affect mood and blood sugar, please run it by your pharmacist or doctor first.
