These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Indole-3-carbinol interacts with whole categories of medication. Cyproterone Acetate, Ethinyl Estradiol falls into 2 of those categories, so it can interact with Indole-3-carbinol in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Cyproterone Acetate, Ethinyl Estradiol
Cyproterone Acetate, Ethinyl Estradiol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Indole-3-carbinol interacts with.
Animal research shows that indole-3-carbinol induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Diane-35 and indole-3-carbinol (I3C). This supplement may speed up a liver enzyme called CYP1A2, which is one of the pathways that helps clear the estrogen part (ethinyl estradiol) of your pill. If that enzyme runs faster, the hormone levels in your blood could drop a little, and in theory that might make the pill slightly less reliable, including for its acne and birth control effects. You might also notice more breakthrough spotting.
I want to be straight with you though: this is based only on lab and animal data, not real human studies, so it's a soft signal rather than a proven problem. Everyday broccoli and cabbage are very unlikely to matter, but concentrated I3C capsules are a different story. Before adding it, please run it past your pharmacist or doctor.

ESTROGENS
2 of 2Cyproterone Acetate, Ethinyl Estradiol also falls under ESTROGENS.
Preliminary clinical and in vitro evidence shows that indole-3-carbinol has antiestrogenic activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about Diane-35 (Dianette): it contains ethinyl estradiol, an estrogen, paired with cyproterone acetate, and it's often used to manage acne, excess hair growth, and as a contraceptive. The concern with indole-3-carbinol is that it may nudge how your liver processes estrogen and could act in an anti-estrogen way. In theory, that might blunt some of the benefits you're taking this pill for.
I want to be honest about how soft this evidence is. It comes mostly from lab and animal studies, not solid human trials, so the real-world impact is uncertain. Eating broccoli or cabbage is not a worry. A concentrated supplement is a different story, so please run it by your pharmacist or doctor before combining the two.
