Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Indole-3-carbinol interacts with.

Theoretically, indole-3-carbinol might increase the metabolism of CYP1A2 substrates and lower serum concentrations.

Here's the gist for ondansetron (Zofran) and indole-3-carbinol. Your liver uses several enzymes to clear ondansetron, and one of them is called CYP1A2. In animal studies, I3C seems to speed up (induce) that enzyme. The theory is that this could clear ondansetron a little faster and lower its levels in your blood, which might make it slightly less effective at controlling nausea and vomiting.

I want to be honest about how soft this evidence is. It comes from lab and animal research, not real-world human studies, so we don't actually know if it matters in people. Eating broccoli or cabbage at dinner is very unlikely to be a problem; a concentrated daily supplement is the bigger question mark. If you rely on Zofran during chemo or after surgery, just run the supplement by your pharmacist or doctor first.

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
