Lonafarnib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist for Zokinvy (lonafarnib). Your body relies on a liver enzyme called CYP3A4 to break this medicine down and keep its levels in a safe range. In lab dish studies, neem leaf extract seems to slow that same enzyme. If that held true in people, lonafarnib could build up higher than intended, which may mean more of its usual side effects like nausea, vomiting, diarrhea, poor appetite, and fatigue.

I want to be honest though: this concern comes only from test-tube research, and it hasn't actually been seen in real patients. Lonafarnib is also a drug that doctors monitor closely, so I wouldn't add or stop neem on your own. Please loop in your prescriber or pharmacist first.

Here's the short version on combining neem with Zokinvy (lonafarnib). Lab studies suggest neem leaf extract may slow down an enzyme called CYP2C9, which is one of the pathways your body uses to clear lonafarnib. If that pathway slows, more of the drug could linger in your system, and that may mean stronger effects and a higher chance of side effects like nausea, vomiting, diarrhea, loss of appetite, or fatigue.

I want to be honest about the evidence though: this is only seen in test-tube research, never reported in actual people, so it's mostly theoretical right now. Still, lonafarnib is a serious medicine for a rare condition, so I wouldn't add a concentrated neem supplement without first checking with your pharmacist or doctor.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
