These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Padang Cassia interacts with whole categories of medication. Trandolapril, Verapamil falls into 2 of those categories, so it can interact with Padang Cassia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
HEPATOTOXIC DRUGS
1 of 2Why this applies to Trandolapril, Verapamil
Trandolapril, Verapamil falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about Tarka (the trandolapril and verapamil combo): this isn't about the spice changing how your medicine works in your body. It's about your liver. Cassia cinnamons like Padang Cassia naturally contain coumarin, and in large, concentrated supplement doses coumarin has been linked to liver irritation in sensitive people. Verapamil itself can occasionally bump up liver enzymes, so stacking two things that lean on the liver is the worry here.
To be fair, this is a theoretical concern based on pharmacology, not strong human proof, and sprinkling cinnamon on your oatmeal is not the issue. It's the concentrated capsules taken daily that could matter. If you take Tarka, just check with your pharmacist or doctor before starting a cinnamon supplement.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Trandolapril, Verapamil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Tarka pairs trandolapril with verapamil, and that verapamil half is the part to watch here. Verapamil is cleared by an enzyme in your liver and gut called CYP3A4. Lab studies suggest a concentrated extract of Padang cassia can slow that enzyme down. In theory, if verapamil isn't broken down as quickly, more of it could build up in your blood, which may lead to stronger effects like a lower heart rate, dizziness, low blood pressure, constipation, or swollen ankles.
The good news: this is based only on test-tube data, not real people, so it's a soft signal. Sprinkling cinnamon on your oatmeal is no concern. If you're thinking about a daily cinnamon supplement, just run it by your pharmacist or doctor first.

From our pharmacist’s overview on the Trandolapril, Verapamil page
“This is one of the supplement lists I would want a patient to read closely, not skim. Tarka combines trandolapril and verapamil to lower blood pressure, and most of these interactions pull on that same lever, either dropping pressure further than intended or working against the medicine altogether.”

About Padang Cassia
What is Padang Cassia?
- Adding flavor to food
- Digestive upset
- Blood sugar support
- Antimicrobial purposes
- Antioxidant support
Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.
