Vincristine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist: your body relies heavily on a liver enzyme called CYP3A4 to break down and clear vincristine. In lab dish (test tube) studies, an extract of Padang cassia slowed that same enzyme down. If that were to happen in a real person, vincristine could build up more than expected, and that's the concern with this drug because its nerve-related side effects are dose-sensitive. We're talking about things like tingling or numbness in the hands and feet, constipation, jaw or muscle pain, and weakness.

I want to be honest about the strength of this, though. It's only based on laboratory research, not people, so it's theoretical. Sprinkling cinnamon on your oatmeal is not the worry. Concentrated cinnamon capsules are. While you're on chemo, please clear any supplement with your oncology team first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

Vincristine Sulfate Injection is approved to treat acute leukemia. It is also used as part of combination chemotherapy regimens for Hodgkin's disease, non-Hodgkin's malignant lymphomas, rhabdomyosarcoma (a soft-tissue cancer), neuroblastoma (a cancer of nerve tissue), and Wilms' tumor (a kidney cancer in children).

Because it works best alongside other cancer medicines, vincristine is almost always given as part of a multi-drug treatment plan rather than on its own.
