Why this applies to Linagliptin, Metformin Hydrochloride

Linagliptin, Metformin Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Red Yeast Rice interacts with.

Theoretically, drugs that inhibit the CYP3A4 enzymes might increase levels of lovastatin from red yeast rice.
Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the good news: your Jentadueto XR (linagliptin plus metformin) isn't really the player in this interaction. Neither of those two diabetes medicines depends much on the CYP3A4 enzyme, and they don't block it in any meaningful way. So they aren't going to push up the lovastatin that's naturally found in red yeast rice.

The flag on file is really about CYP3A4 inhibitors raising lovastatin levels, and your combo pill just doesn't fit that role strongly. This is a theoretical, lab-based concern, not something seen causing real problems in people on these drugs. Still, red yeast rice acts like a statin, so muscle aches or liver strain are worth watching. Please run it by your pharmacist or doctor before adding it.

“Someone taking Jentadueto XR (linagliptin and metformin) for type 2 diabetes is often the same person eyeing fenugreek, bitter melon, or gymnema, and that overlap is why this page deserves real caution. About Red Yeast Rice

What is Red Yeast Rice?

Red Yeast Rice
  • High cholesterol
  • Heart health
  • Metabolic syndrome
  • Statin alternative

Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. Because of this, it carries the same risks as statin drugs, and the amount of active ingredient varies widely between products. Always talk to your pharmacist or doctor before using it, especially if you have liver problems or take other medicines.
